This program provides a county-managed healthcare option as a last resort for time-sensitive specialty medical care. It partners with local safety net clinics and medical experts to offer various specialty healthcare services, such as dermatology, gastroenterology, OB/GYN, nephrology, neurology, orthopedics, and urology.
Intake Process: Applications can be made via phone or by visiting in person for an appointment.
Costs: There is no application fee, but there is a $12 co-pay for office visits, a $4 co-pay for each prescription, a $45 co-pay for Emergency Department visits, and a $15 co-pay for retinal eye scans.

- Eligibility is open to uninsured or underinsured residents of Santa Cruz County, regardless of immigration status, who have lived in the county for at least 6 months and whose income is between 139% and 160% of the Federal Poverty Level, requiring a referral for specialty care from a primary care provider, with a review of financial resources and Medi-Cal eligibility.
