Housing For Health Partnership

Coordinated Entry serves as a structured method for linking individuals who are experiencing homelessness with available community assistance. This approach is crucial for ensuring fair and equal access to the limited housing resources, fulfilling both Federal and State mandates. The objectives of the new Coordinated Entry System include: (1) Enabling connections to mainstream and community services for as many individuals facing homelessness as local resources permit; (2) Simplifying the process for matching individuals to the scarce housing resources within the Housing for Health Partnership network (CoC); and (3) Prioritizing assistance for households facing the most significant challenges in securing and maintaining housing, as well as those at the highest risk for personal health and safety issues. H4HP Connectors may function as part of outreach teams, drop-in centers, multi-service centers, or other programs aimed at assisting individuals who are homeless or at risk. They engage with those experiencing homelessness, build relationships, enroll participants in HMIS programs, and gather participant data. Connectors provide problem-solving support to all participants and may carry out a Housing Needs Assessment (HNA). The HNA equips Connectors and participants with the necessary information to collaboratively develop an individualized Housing Action Plan and helps identify which participants qualify for and are prioritized for H4H-supported housing and services. Only those households deemed highest priority are placed in the queue for the limited housing resources available through the system.
