This program offers medical, educational, and psychosocial support to children with chronic health issues who struggle in a conventional school setting due to their medical needs or difficulties related to their diagnosis.

- Participants must be aged 7 to 18, have chronic medical conditions, and be recipients of Medicaid benefits. The program is available nationwide.

- Must provide: Name of patient's primary care doctor Guarantor's legal name (individual financially responsible) Guarantor's ID card Guarantor's health insurance card containing policy numbers Guarantor's insurance claims address and customer service phone number (should also have pre-approval from insurance concerning child's appointment)
