This program provides support for low-income individuals who are either in need of or at risk of being placed in nursing homes. It aims to help these individuals continue living in their homes or other community environments as alternatives to nursing home care.
Intake Procedure: For more details, you can either call or visit in person.

- To qualify, individuals must: be at least 18 years old, have a monthly income not exceeding $2,901, and possess countable assets of $2,000 or less.
