Silver Thread Public Health District

The program provides assistance to individuals and families in completing applications for several state benefit programs, such as Colorado Works (Temporary Assistance for Needy Families), Old Age Pension (OAP), Aid to the Needy and Disabled (AND), Food Assistance Program (SNAP), Health First Colorado (Colorado’s Medicaid Program), Colorado Child Care Assistance Program (CCCAP), and the Low-income Energy Assistance Program (LEAP).

To begin the process, individuals can either call to set up an appointment or submit their applications online at www.colorado.gov/peak.

Once the necessary paperwork is completed, case workers from the Gunnison Department of Human Services will assist individuals and families.
