The program assists families with children who have special health care needs by providing tailored care coordination and facilitating access to specialty care through referrals. It also helps families locate screenings, clinics, medical services, community resources, financial assistance (including help with insurance), support groups, and transition services.
Intake Procedure: Families can call or visit in person for a consultation.
