To apply, individuals can call for further details, visit the Colorado PEAK website, or apply in person. Applications are accepted online, via phone, in person, or through the mail.
PEAKHealth is designed for Medicaid and CHP+ members who have a Colorado.gov/PEAK account. With the PEAKHealth mobile app, members can easily update their information and access vital health details directly from their smartphones, available for free on the Apple iTunes Store and Android/Google Play App Store. Colorado PEAK accounts can be created and accessed at https://coloradopeak.secure.force.com/. Medicaid members can also find information on providers, services, and coverage through the Health First Colorado website: healthfirstcolorado.com/
