- Applicants must be licensed small food business owners in Mesa County and maintain good standing with the Colorado Secretary of State.

- Must provide: Certificate of good standing from the Colorado Secretary of State License from Mesa County Public Health Department, CDPHE, and FDA (as applicable) Proof of certified food protection manager certification for one owner $2M general liability insurance policy
