Intake Procedure: You can apply by calling, visiting in person, or submitting an application online. Those applying for Medicaid do not need to fill out a new application for CHP+ for their family members.
Travel/Location: PEAKHealth is available for Medicaid and CHP+ enrollees who have an account on Colorado.gov/PEAK. With PEAKHealth, members can maintain their information easily and access vital health details from their mobile devices by downloading the PeakHealth app available for free on the Apple iTunes Store or Android/Google Play App Store.
