This outreach program offers food delivery to those who are unable to reach local food pantries because of chronic illnesses, disabilities, or other situations. *PLEASE NOTE: A request for food delivery must be authorized by a case manager from Social Services or another qualified professional.
Intake Procedure: The application process must be initiated by an authorized professional, such as someone from the Department of Social Services. Please call for more information.
Fees: Clients are encouraged to call for detailed fee information.

- Residents of Putnam and Westchester Counties who are low-income, homebound due to illness or disability, elderly on fixed incomes, disabled, critically ill, veterans, or those with chronic illnesses requiring a specialized diet.
