Gentiva Health Services

This program is dedicated to assisting patients aged 65 and above in attaining optimal health and independence by delivering specialized medical services directly to their homes. It includes a variety of skilled healthcare services such as nursing, physical therapy (PT), occupational therapy (OT), speech therapy (ST), social services, dietary consultations, and traditional in-home skilled services that cover medication management, wound care, and education on managing various diseases and conditions like diabetes, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), and urinary catheter care. Additionally, it provides four specialized services: Fall Prevention (Safe Strides Program), Orthopedics, Low Vision, and Neuro Rehabilitation. The program accepts Medicare, Medicaid, and other health insurance plans. Services are offered in English.

Intake Procedure: The process begins with the submission of basic patient information via fax or phone.
