The application can be accessed online or at the DMV Office, and it requires a doctor's completion of the medical certification section. To qualify, applicants must demonstrate an inability to walk 200 feet unassisted or without considerable difficulty due to eligible impairments. (Refer to the application for further details.) Languages available: English.
Intake Procedure: The application can be completed online or obtained at the DMV. Use Form # ITD 3392.
