This program provides comprehensive primary care services tailored for seniors, including telehealth appointments for cardiology, nutritional guidance, laboratory tests, preventive health screenings, routine blood tests, and testing for influenza types A and B, COVID-19, and strep throat. It also includes skin cancer checks, urinary incontinence evaluations, and vaccinations for flu, shingles, tetanus, and pneumonia. Same-day medical appointments may be available, and services are offered in both Spanish and English.
Intake can be done through walk-ins or scheduled appointments. For more information, you can call, email, or visit the website.
Fees are determined on a sliding scale.

- Eligible participants are seniors aged 60 and above who are enrolled in Medicare Advantage Plans or D-SNP Medicaid plans.
