This program provides outpatient mental health counseling and crisis stabilization for individuals aged 18 and above who are dealing with chronic illnesses such as HIV/AIDS and kidney disease/dialysis, as well as issues related to medication adherence and homelessness. Services can be accessed at home, through telehealth, or in-office. Available languages include English.
Intake Procedure: Services are available by appointment; interested individuals should call or email for more information.
Fees: There is no charge for the services.
