Nevada Senior Services

This initiative provides a complimentary program lasting between 30 to 90 days for adults aged 18 and above, including those with intellectual and developmental disabilities, who live alone or with caregivers. It is designed for individuals who have recently experienced hospitalization due to dementia, chronic health issues, or complications from COVID-19, particularly those related to isolation during the pandemic. The primary aim is to lower the rates of hospital readmissions and reduce the necessity for acute care by offering assistance to adults facing memory loss and other challenges, as well as their caregivers, with essential resources to address their health and social requirements while also delivering respite and educational support.
