Sahara Home Health

This program assigns a physical therapist to conduct visits at home, delivering tailored treatment and guidance that includes home safety assessments, caregiver training, enhancing patient mobility, transfer training, therapeutic exercises, post-operative support, gait and balance training, and instruction on the correct use of assistive devices. Available languages include English, Spanish, and Tagalog.

To initiate the service, a call is required for an in-home evaluation with a doctor or medical personnel.

Costs are fully covered by Medicare.
