Comprehensive case management aids seniors in formulating a personalized care plan that addresses their physical, psychological, social, and financial requirements. Case Managers are responsible for coordinating the necessary services, and they monitor cases over the long term to ensure that seniors receive the services they need. Additionally, Case Management services offer help with completing paperwork for various programs. Languages available: English.
