Lyon County Human Services

This program delivers support and case management for individuals classified as homeless or at risk of homelessness who are dealing with serious mental health conditions or physical illnesses that hinder their ability to work. It encompasses the entire social security application process, including assistance with applications, monitoring, and acting as the authorized representative. Additional services provided include help with community referrals, scheduling appointments, transportation support, interactive case management, volunteer opportunity assistance, and housing and income stabilization referrals. Translation services are available, with English as the primary language.
