Intake Procedure: No appointment is required; walk-ins are welcome.

- *Photo ID for both taxpayer and spouse Social Security Card or Individual Taxpayer’s Identification Number (ITIN) *Income Statements (W2, W2-G, 1099) or Income Transcripts *Name, SSN/EIN of day care provider and amount paid *Account and routing numbers of checking or savings accounts *Health coverage information for each family member (1095 A, B, C) *Copy of last year’s federal and state returns, if available
