This program provides support in the form of access or care coordination for individuals aged 60 and above, as well as their caregivers, who are dealing with reduced functioning capacities or other conditions that necessitate services from a formal provider. Available languages include English.
Intake Procedure: Available for walk-ins or scheduled appointments.
Fees: No charges apply.

- Individuals aged 60 and older who are functionally impaired and unable to carry out Activities of Daily Living, with additional consideration for those struggling with Instrumental Activities of Daily Living.
