This initiative provides advocacy, information, and referral services for children from birth to 21 years who have, or are suspected of having, serious or chronic physical, developmental, behavioral, or emotional conditions that necessitate health or related services exceeding what is typically needed by children. It also offers case management support for families to engage with healthcare providers, learn about available programs, advocate for special services with school districts, make referrals for specialized healthcare, and access family support services.
Intake Procedure: Call.
Fees: Medical care is provided at no cost. A family contribution is required for orthodontic services; please call for additional details.
