Care Managers assist in connecting clients and their families with services to address various medical, social, emotional, behavioral, and fundamental needs for youth up to 21 years old with active Medicaid and specific health conditions such as chronic physical or mental health issues, HIV/AIDS, or Complex Trauma.
Intake Process: Contact the Care Coordination Intake line at 716-378-2786. Referral forms can be submitted via email to carecoordination@ccwny.org.
Costs: There are no fees for these services.
