Single Point Of Access (Spoa) - Cattaraugus County Department Of Community Services

This initiative enhances services by integrating monitoring and advocacy to help maintain or improve an individual’s quality of life within the least restrictive environment, while considering the continuum of care. It provides a Single Point of Access (SPOA) for linking to community agencies and providers. SPOA is a process aimed at achieving specific outcomes rather than a service itself. The initial focus of SPOA will be on individuals with significant needs. It will coordinate various services, which may encompass: living arrangements, educational and vocational support, physical health, family services, mental health, self-care, economic assistance, transportation, social services, substance abuse treatment, Assisted Outpatient Treatment (AOT), and Intensive Care Coordination Services (ICM). The SPOA Committee is made up of representatives from various organizations, including the Cattaraugus County Department of Social Services, the Probation Department, and several health and community service agencies, among others involved in SPOA referrals. Referrals can be made by community or agency providers as well as by individuals. Most expenses are typically covered by Medicaid.
