Delivers EISEP Case Management, Personal Emergency Response Systems (PERs), In-Home Contact and Support, Health Insurance Information Counseling Assistance Program (HIICAP), Home Delivered Meals (HDM), Congregate Meals, Nutrition Counseling, Nutrition Education, Information and Assistance, NY Connects Options Counseling, Afternoon Trips and Transportation, and recreational and educational activities at the Senior Center. A partnership with the Center for Elder Law & Justice provides legal aid.
Intake Procedure: Accepts calls, emails, and walk-ins.
Fees: Suggested contributions differ based on the services rendered.
