Senior Life Matters - Lutheran

Support is offered for the coordination of care for seniors residing in their own homes and their families. The program helps connect individuals with the right resources and assesses eligibility for SSI, Medicaid, Lifeline, Food Stamps, and telephone discounts. It also assists with the necessary paperwork for entitlement programs like Medicare, Medicaid, and Food Stamps, along with insurance counseling. Educational resources and counseling related to Medicare and its associated products are provided. Transportation to medical appointments is available for a fee that includes an hourly rate plus mileage. Additionally, assistance is offered in choosing alternative living arrangements, keeping the older adult's preferences in focus.

Intake Procedure: Registration is done via phone call.
