This initiative provides integrated housing support for individuals with disabilities who are at risk of homelessness. Housing units are available for Medicaid recipients aged 55 and older who need help with daily living activities, as well as for individuals aged 18 and older eligible for services through the NYS Office for People with Developmental Disabilities (OPWDD) and disabled military veterans. On-site staff members advocate for residents, link them to community services, offer training in independent living skills, and organize social and recreational activities.
Fees: There are no associated fees.
