This initiative provides a centralized intake system for both adults and children facing substantial behavioral health challenges, allowing them to access additional community support and services. It targets individuals with notable behavioral, medical, or social risk factors that can be managed through comprehensive care. There are three categories of SPOAs: Children's, Adult Case/Care Management, and Adult Housing. The SPOA process aims to identify those who require services the most and oversees their access and use of these services.
Intake Procedure: Visit the website to obtain referral forms. For Care Management Referrals for Medicaid-eligible individuals, fax the completed forms to (585) 613-7670; for all other referrals, fax to (607) 737-5563.
Fees: There is no charge for the intake process.
