A Health Home is not a physical location but a service that provides complimentary care management to Medicaid recipients, helping to address their medical, mental health, and substance use disorder requirements. BestSelf Health Home Services aim to facilitate collaboration among all parties involved in a person's care, ensuring they share essential information to support recovery. Besides physical health services, the Health Home also aids in coordinating social service needs.

Fees: Services are available only to Medicaid recipients.

- To qualify, individuals must be Medicaid recipients aged 18 or older residing in Erie County, including those already enrolled in a Managed Care Organization or those with both Medicaid and Medicare. They must meet at least one of the following criteria: have two or more chronic health conditions such as asthma, diabetes, heart disease, mental health, or substance use disorders; have a significant mental illness; or be living with HIV/AIDS. The service area includes Erie and Niagara Counties.
