Adult Services - Community Services For Every1

The initiative focuses on empowering individuals with developmental disabilities, as well as youth and adults facing chronic health issues, complex mental health challenges, and those who have survived domestic violence, to enhance their independence. Adult Services encompass: Day Services, which include both site-based and community-based day habilitation, along with the Individual Retirement Experience (IRE) tailored for individuals aged 50 and older; Employment Services aimed at skill development and connecting individuals to competitive job opportunities; Self-Directed Services that allow individuals to choose their staff, supports, and services; Health Homes providing comprehensive case and care management; In-Home Services offering training for adults with deficits in independent living skills to enhance or maintain their capabilities and lessen dependence on family caregivers; Behavioral Services that include home and community-based interventions; Intensive Supports for family relief; Representative Payee Services designed to help individuals improve their financial skills towards independence; Housing Subsidies for eligible clients; Residential Services; Transportation; a Family Reimbursement Program to assist families caring for individuals with developmental disabilities; an Alternatives to Incarceration Program for those navigating the criminal justice system; Raise the Age Services to help youthful offenders develop necessary life skills and pro-social behaviors; and Project C.O.A.C.H., which provides counseling, referrals, outreach, and intervention for individuals and families affected by domestic violence.
