This initiative aims to enhance the coordination of care for children and youth with special health care needs by connecting families to community resources and referring them to relevant programs. It includes advocacy efforts with healthcare providers and school districts, linking families to medical professionals and specialists, as well as providing support through playgroups and assistance during the transition to adult services.
Intake Process: Interested individuals should call for application details. Parents, caregivers, school personnel, medical providers, and friends or family members of children and youth up to 21 years old can apply on their behalf, while youth aged 18-21 can submit applications independently.

- Children and youth up to 21 years old residing in Erie County with serious or chronic medical, physical, behavioral, emotional, or developmental conditions are eligible, with no diagnosis required for application.
