National Family Caregiver Support Program - Erie County Department For The Aging

This initiative provides caregivers with crucial information regarding the services accessible for their family members or friends and the steps to acquire them. Staff members conduct Options Counseling to connect caregivers with further information, resources, and programs available in Erie County, enabling them to make informed choices that reflect their loved ones' preferences, strengths, values, and needs. The services offered include: -Social Adult Day Services (SADS): Assistance and funding for contracted SADS programs, allowing caregivers to take a much-needed break while providing enriching activities for their loved ones. -Overnight Respite: Financial support for overnight care (typically up to 5 nights) in a skilled nursing facility or assisted living, giving caregivers the opportunity to go on vacation. -Caregiver Counseling: Access to funding for six (6) sessions with a professional support counselor. -Alzheimer's Disease Caregiver Support Initiative: Collaboration with the Alzheimer’s Association to offer supportive services to caregivers of individuals with Alzheimer’s Disease or memory loss. Services encompass care consultation, short-term respite, and connections to safety devices and legal assistance. -Caregiver Community Information and Resource Packet: Comprehensive information on respite and support services provided by various agencies in Erie County. -Kinship Caregivers: Access to community and legal resources for older adults caring for a grandchild or other children not biologically their own. *Services are subject to the availability of grant funds and eligibility criteria.
