This program offers case management for children aged 0-21, assisting them in accessing social and medical resources within the community, as well as support with the Department of Social Services and other local services.
Intake Process: You can either call or fill out a referral form available on the website.
Cost: No fees are charged.

- Eligible participants are children aged 0-21 who qualify for Medicaid and have either two or more chronic conditions or one specific qualifying condition (such as serious persistent mental health issues or HIV/AIDS). The program serves the WNY area.
