This initiative delivers support, counseling, training, and essential life skills to those dealing with mental health and substance use issues. Services are accessible at home, within the community, or via telehealth, and can complement other behavioral health services currently being utilized. The offerings include Peer Support Services, Psychosocial Rehabilitation, Counseling, Family Support & Training, as well as Education and Job Support Services.

- Eligible participants are individuals aged 21 and older who are enrolled in a Managed Medicaid plan and have a qualifying behavioral health diagnosis. The service area includes WNY and nearby regions.
