This program provides care management for individuals aged 0-21 to help them navigate medical, behavioral, and physical health services, including scheduling medical appointments. Additionally, it offers educational assistance with IEP and 504 Plans, along with support for transportation issues.
Intake Procedure: Contact for referral.
Fees: No cost.

- Eligibility is determined by the program; individuals must have two chronic conditions or a history of complex trauma. The service area includes Cattaraugus, Chautauqua, Erie, and Niagara Counties.
