This service serves as a replacement for the Targeted Case Management Services. The agency collaborates with Adirondack Health Institute (AHI) to offer support and connections aimed at assisting individuals with their mental and physical health care requirements. Funding is provided by OMH, and there is a contract with Health Home and DOH.
