This initiative provides support without income restrictions for individuals aged 18 and above who have physical or mental disabilities that hinder their ability to fulfill basic needs such as food, clothing, shelter, or medical care. It also assists those who cannot safeguard themselves from neglect, abuse, or dangerous situations and lack responsible help. The Personal Care Services Aide Program offers varying levels of aid with personal hygiene, dressing, and feeding, along with nutritional and environmental support, as well as health-related tasks. These services are deemed essential for maintaining the patient's health and safety at home, ordered by a physician following a needs assessment by a qualified professional, and supervised by a registered nurse. The Consumer Directed Personal Assistance Program allows patients to manage their own services, including hiring, training, and supervising their attendants. For intake, a doctor's order is necessary. Anyone concerned about an individual struggling with basic needs is encouraged to call 585-344-2580 ext. 6484 to make a referral. Individuals can also reach out for assistance if they recognize their own need for help. All adult service assessments are reviewed for Adult Protection eligibility, regardless of the assistance type requested. Typically, a caseworker will conduct a home visit within three business days of receiving the referral, or within 24 hours if the situation is life-threatening.
