The program delivers Intensive Case Management (ICM) for children and adolescents, ages 5-17, who are facing serious challenges in community functioning. It aims to aid families in their efforts to support their children, thereby decreasing the likelihood of hospitalization and out-of-home placements. Services include weekly home visits, daily living assistance, advocacy, referrals, crisis intervention, and service coordination.
Intake Procedure: Clients are encouraged to contact the agency for further information.
Fees: Medicaid is accepted.
