Community health nurses deliver educational resources and support to pregnant women, postpartum women, and pediatric patients, including medical monitoring, health education, pre-and post-natal care, and case management for high-risk pregnancies. They also run an immunization clinic and conduct lead poisoning screenings. Additional services include the Pediatric Home Visit Program for children with acute or chronic health issues, the Early Intervention Program for children with disabilities or developmental delays, and the Infant Child Health Assessment Program/CHAP, which provides screenings for physical, social, and language development.

Fees: Some services may incur charges, often based on a sliding scale.
