This initiative provides onsite health and wellness services and coordination for adults aged 55 and above who are homeless, have a chronic health condition, and possess active Medicaid. It includes a rental subsidy for eligible participants living at one of the two designated locations under the ESSHI program. Additionally, transportation is available for participants to attend scheduled shopping trips and off-site health and wellness activities sponsored by Supportive Housing.
