This service provides outpatient medical care to individuals and families, covering areas such as primary and preventive care, pediatrics and family medicine, dental services, obstetric and gynecologic care, geriatrics, podiatry, nutrition, immunizations, speech and hearing screenings, and Medication Assisted Treatment (MAT) for opioid addiction. Additionally, it offers short-term mental health and addiction services, as well as optometry services that include vision testing, glaucoma assessments, eyeglass prescriptions, and diabetic eye screenings.
Intake Procedure: Schedule an appointment by calling.
Fees: Accepts Medicaid and offers a sliding scale based on income, with discounts available up to 75%. Contact for details.
