This program offers care and support to individuals, which includes home visits when necessary, organizing medical treatments, and aiding in locating housing and public assistance.
Intake Procedure: Referrals can be made by family members, social supports, healthcare providers, or through self-referral. For help with the referral process, contact the program supervisor.
Fees: Services are covered by Medicaid, and clients do not incur any charges.
