This initiative delivers Health Home services via a collective of healthcare and service providers. Individuals enrolled in the program collaborate with a care manager to formulate a tailored care plan. The program aids individuals in various ways, including: coordinating care through communication with other healthcare providers, advocating for their needs to access services, navigating the healthcare system, connecting with mental health and substance abuse resources, obtaining necessary medications, and linking to support services for affordable housing, benefits, food, clothing, childcare, and transportation. It also promotes learning about activities that enhance health, setting personal goals, and ensuring that all involved providers are aware of these goals and the care plan.

Intake Procedure: Interested individuals should visit the website to complete and print the application, which can then be mailed or faxed. Fees: There is no service charge for Medicaid recipients.

To qualify, individuals must be Medicaid eligible or active recipients and have either two chronic health conditions (such as asthma, diabetes, heart disease, COPD, obesity, or substance abuse disorder) or a single qualifying diagnosis, which includes serious mental illnesses (like Bipolar Disorder or Schizophrenia), HIV/AIDS, and significant behavioral, medical, or social risk factors that can be managed through care.
