- CHILDREN: From birth to twenty-one years old, must have active Medicaid and two or more chronic conditions. ADULT: Must be 21 years or older, a Medicaid recipient, and have two or more chronic conditions, which may include mental health issues, substance use disorders, asthma, diabetes, heart disease, obesity, and others. Conditions like HIV/AIDS and significant trauma also qualify.
