It serves as a gateway to other community services and programs tailored for seniors, including care management, both short-term and long-term care options, comprehensive in-home assessments, and the development, execution, and monitoring of care plans. Additionally, it connects consumers to a wide range of community services, facilitates transportation arrangements for older adults, offers assistance in evaluating housing options, and provides mental health assessments and guidance. The Community Care Connections Program also visits senior centers monthly to deliver services. For health-related assistance, the Health Homes Program is available. Intake is conducted via phone inquiries, and while donations are welcomed for case management and in-home assessments, specific fee information can be obtained by calling.
