This program facilitates the organization of in-home health care and support services for families, overseen by their personal physician. Home care coordinators are available in all local hospitals, providing extensive nursing services along with rehabilitation and habilitative therapies, including occupational, physical, and speech therapies. Additionally, it offers certified hospice and palliative care, home health services for individuals with developmental disabilities, social casework, mutual support groups for bereavement and caregivers, and volunteer opportunities.
Intake Procedure: Referrals for home nursing care evaluations are made through a physician, while homemaker-home health aide services can be accessed via agencies or self-referrals.
Fees: Payment options include third-party insurance, Medicare, Medicaid, and private pay.
