Facilitates access to health, social, and long-term care services for individuals who are chronically ill or disabled and prefer to remain in their homes. Services include in-home nursing, therapies, health aides, personal care, adult day health care, private duty nursing, and consumer-directed personal assistance.

Fees: Services are provided at no cost.

- Individuals aged 18 and older who reside in the service area and are eligible for Medicaid, requiring long-term care services for at least 120 days per year.
