This program allows participants or their representatives to exercise decision-making power over services and take on the responsibility of managing those services, thereby enhancing personal choice. It is funded by Medicaid and is available for those who qualify under OPWDD.

Fees: There are no fees involved.

- To qualify, individuals must be at least 2 years old, eligible for Medicaid, have a Medicaid Service Coordinator (MSC), go through the Front Door for OPWDD eligibility, secure a start-up broker, a Financial Intermediary (FI), and a Circle of Support (COS), and opt for HCBS waiver services instead of institutional care.
