This initiative provides housing and support services designed to assist individuals with psychiatric disabilities in maintaining their independence within the community. It is available to residents of Nassau County who are 18 years or older and have a primary psychiatric diagnosis. In some locations, participants must also comply with HUD's homelessness criteria. A referral is necessary, particularly for those involved in the OASAS program who have a primary substance abuse diagnosis and have experienced hospitalization along with homelessness.
Intake Procedure: Clients are encouraged to call the agency for further information.
Fees: A sliding scale is applied based on income, and a referral is required for those with a primary psychiatric disability.
