This program provides residential support for individuals with learning and developmental disabilities, allowing them to receive supervision and assistance in community-based apartments. It aims to foster daily living patterns that encourage personal growth and independence to the fullest extent. The service is available for adults aged 18 and older residing in Nassau and Suffolk Counties who have learning and/or developmental disabilities. Interested individuals should call to arrange an intake appointment.

- Residents of Nassau and Suffolk Counties must have Medicaid, be eligible for the HCBS waiver, and have a documented developmental disability before turning 22.
