Nassau County Department Of Health

This program offers a voluntary developmental assessment and services initiative managed by New York State and locally by the Nassau County Department of Health Office for Children with Special Needs. To qualify for Early Intervention (EI) services, children must be under the age of three and have a confirmed disability or an established developmental delay, as outlined by New York State, in one or more developmental domains: physical, cognitive, communication, social-emotional, and/or adaptive. Evaluations are performed to assess eligibility. A range of therapeutic and support services is available for eligible infants and toddlers with disabilities and their families. Key aspects of the program include: identification and referral of children at risk or suspected of having a disability by primary referral sources; regular developmental screenings and monitoring of at-risk children; service coordination for eligible families; a multidisciplinary evaluation at no cost to parents to determine eligibility; Individualized Family Service Plans (IFSPs) for qualifying children and families; provision of early intervention services in the IFSP at no cost to parents; and delivery of services in natural community settings where children of similar ages typically gather, as much as possible.
